The Caring4Urology website operated by P4, LLC ("P4"), including P4's Health Center and Health Records, are free services (with the exception of the electronic medical record link) to registered users of Caring4Urology.com. PLEASE READ THESE TERMS OF SERVICE CAREFULLY BEFORE USING THIS SITE. By using these services, you signify your agreement to these terms. If you do not agree to these terms, you should not use these services.

Not Medical Advice 

Caring4Urology's content is not intended as a substitute for the advice provided by your own physician or health care provider and does not take your individual health situation into account. You should not use the information on Caring4Urology as a means of diagnosing a health problem or disease, or as a means of determining treatment. Caring4Urology does not provide emergency medical services or medical advice. In the event of a medical emergency, immediately contact a doctor or call 911.

Children 

This Site is not intended for children under the age of 18. We do not collect personally identifiable information from any person we know (or reasonably believe) is under the age of 18.

Privacy Statement

Caring4Urology has created this Privacy Statement to demonstrate our commitment to protecting your privacy. The following text discloses our information-gathering practices with respect to Caring4Urology's Health Center and Health Records users. This text explains how your personal information is used, protected and, in certain limited instances that do not violate your privacy, disseminated.

Type of Information Gathered 

The Caring4Urology registration form requires users to give us contact information (name and e-mail address), personal health information (gender, date of birth, and additional health information), and demographic information (ZIP or postal code). Users may elect to store additional health information on our Site. Storing additional health information on the Site gives the user access to his own personalized information, Health Tracking and Health Records.

How the Information is Used 

Caring4Urology pledges to protect the privacy of all of our registered users.

We use contact information from the registration form for internal purposes, to send the user news, information about new Caring4Urology site features, services or updates, information about our company, and promotional material or offers from our corporate partners. You will be given the opportunity to decline these services at the point of registration, and you may opt-out of receiving future mailings (see the Choice/Opt-Out section following).

Demographic and personal health information is used to customize the visitor's experience at our Site, showing them content we believe would be of interest to the user, and displaying the content according to the user's preferences. Your health information is password-protected to prevent access by others.

Caring4Urology will not sell or forward to third parties any individually identifiable personal information, e.g., your e-mail address, name, etc., that you supply during the registration process unless you have specifically opted to provide such information to specified third parties or allow third parties access to it. Caring4Urology may, however, provide your personal health information and that of other registered users in aggregate form to third parties, but never in such a way that it violates your privacy. This means we will never provide anyone's individual health records to any third party without express consent, although we may provide advertisers with information about the prevalence of certain health conditions in the overall group of Caring4Urology subscribers, and similar aggregate information.

Though we make every effort to preserve user privacy, we may need to disclose personal information when required by law wherein we have a good-faith belief that such action is necessary to comply with a current judicial proceeding, a court order or legal process served on our Company.

All personal, identifiable information is retained by P4 indefinitely, unless a user requests that the information be deleted. If you would like your information deleted, you can use the “delete all my records” link on the edit profile page. Cookies 

An HTTP cookie is a small packet of information sent by a server to a browser and then sent back by the browser each time it accesses that server. We use cookies to provide us with information relating to the sources of our Site traffic. Most Web browsers allow you to reject cookies but are initially set to accept them. You can modify each browser’s security settings so you can approve or reject cookies from each individual website. If you reject cookies from our Site, some parts of our Site may not work properly.

Some of our sponsors and advertisers may use cookies whenever you view specific pages on our Site. Security 

Caring4Urology is committed to privacy protection and utilizes the most current security tools available to ensure the privacy and confidentiality of all registered users. An individual user ID and password protect each Caring4Urology account so that no one other than the user and those to whom the user has granted access can access its features. In addition, all information is protected through secure socket layer (SSL) software, which encrypts the data to protect it during transmission. Caring4Urology maintains this information, encrypted, on a secure, protected server. Caring4Urology's Health Center and Health Records are certified by Thawte (http://www.thawte.com) as secure health information tools.

Passwords 

P4 has applications that allow you to record and store information. You are responsible for protection of your own username and password. P4’s Health Record allows a user to permit a healthcare provider to view the user's information by a secure link or by creating a unique username and password for that provider. It is your responsibility to (1) control the dissemination and use of activation codes and passwords; (2) authorize, monitor, and control access to and use of your P4 LLC account and passwords; (3) inform P4 of any difficulty in deactivating passwords or removing your records. You grant P4 and all other persons or entities involved in the operation of the Site the right to transmit, monitor, retrieve, store, and use your information in connection with the operation of the Site. P4 cannot and does not assume any responsibility or liability for any information you submit, or for use or misuse, by your or persons authorized by you, of information transmitted or received using P4 tools and services.

Choice/Opt-Out; Correct/Update 

Caring4Urology provides users the opportunity to opt out of receiving communications from us and/or our corporate partners when they register to use of our services.

Caring4Urology provides users the opportunity to change or modify information previously provided.

Specific information regarding opt-out or correction of information is provided at appropriate locations within the Site.

Your Responsibilities as a Caring4Urology User 

You are responsible for obtaining and maintaining the equipment necessary to access and to use Caring4Urology's services (computer hardware, telephone line, etc.). You also are responsible for maintaining the confidentiality of your user identification and password. You should contact Caring4Urology immediately in the event that unauthorized use of your account occurs.

Message Boards and Chats 

You agree that you will not:

Submit or post materials or URLs that are obscene, unsavory, defamatory, unlawful, threatening, abusive, slanderous, hateful, or potentially embarrassing to any person or entity, as determined by P4 at its discretion;

Use Message Board and Chat areas in any way that would violate local, state, national, or international laws;

Submit or post materials or URLs that compromise or endanger the rights, privacy or publicity rights of others;

Submit or post advertisements or solicitations for business whether by text or by a referring URL;

Submit or post pyramid schemes or chain letters, whether by text or by a referring URL;

Impersonate any other another person, real or fictitious;

Upload viruses or other harmful computer code, whether by text or by a referring URL;

Permit any other person to use your user identification and password for posting or viewing comments

Post the same text more than once;

Collect and/or forward specific information about others, including email addresses, without their consent;

Interfere with or interrupt a specific discussion by posting comments that are not related to the topic being discussed (unless it is specifically allowed in that particular forum) or in any other manner;

Engage in any activity or conduct on the Site that interferes with, dissuades, prevents or restricts any person or persons from using the Message Boards, Chats or any other areas of the Site, or which may expose P4 or any of its customers or suppliers to liability and/or harm of any type.

P4 may, at its own discretion, do any or all of the following:

Record, monitor and use the dialogue in public chat rooms for any public or private purpose;

Remove any communication(s) in the public areas, for any reason;

Filter and/or refuse to post any submission, question or communication for any reason;

Limit the number of questions that can be asked of an expert;

Block any user's access to any or all Message Boards and Chats and/or the entire Caring4Urology.com Site for any violation(s) of these Terms and Conditions or for any reason.

P4 or its licensors have no liability or responsibility to users of the Caring4Urology.com Site or any other person or entity for performance or nonperformance of the aforementioned activities.By submitting any question, message or content to the Public Areas, you agree that such submission is non-confidential for all purposes. You also understand that submitted questions may not be answered due to volume.

Online Surveys and Polls 

We may run surveys on our Site in which we ask visitors for answers to questions. We keep track of each individual’s responses, but never disclose this information to anyone except in aggregate form. This means that no third party will ever know an individual’s answers. We may use this information to submit secure messages to your message center (if you’ve opted in) about products and services and possible discount opportunities that you may want to investigate.

Messages and Reminders 

The Message Center is a secure area where you can send yourself reminders for Drug Refills, Appointments and more. You can opt to send these messages to your cell phone, or email which is not a secure channel of communication, so please use this service with caution. P4 shall not be held responsible or liable for any message or reminder you choose to send through either of these insecure channels.

Content and Services 

The materials on our Site are copyrighted by us and/or other third parties. You may download and reprint a single copy of the materials posted on Caring4Urology.com for your own personal, noncommercial use only, provided you include all applicable notices, disclaimers and copyright information. Any other use of the materials is strictly prohibited without our prior written consent of P4 or any other of its third party vendors, licensors, suppliers and customers.

Without limiting the foregoing, P4, its licensors, and its suppliers make no representations or warranties about the following:

The accuracy, reliability, completeness, or timeliness of the content, software, graphics, links, or communications provided on or through the use of the Caring4Urology.com Site or P4;

The adherence to any government regulations that require disclosure of information on prescription drug products or the approval or compliance of any software tools with regard to the content contained on the Caring4Urology.com Site.

We do not operate, control, supply, endorse, warrant or guarantee any information, products, services or merchandise that is presented on our Site. We also do not warrant or guarantee that files available for downloading through the Site will be free of infections or viruses, worms, Trojan horses or other code that contains contaminating or destructive properties.

We, and our content providers, cannot and do not guarantee or warrant against errors, omissions, delays, interruptions or losses, including loss of data. Users of this Site are responsible for maintaining a means external to Caring4Urology.com for the reconstruction of any lost data.

Users agree to indemnify and hold P4, any parent or subsidiary companies or organizations, potential successors, their officers, directors, employees, agents, licensors, and suppliers, harmless from and against any liabilities, claims, actions, demands and settlements, including without limitation, reasonable legal and accounting fees, resulting from, or alleged to result from, user's violation of these Terms and Conditions.

Other 

This agreement shall be governed by and construed in accordance with the laws of the State of Maryland, without giving effect to its conflict of laws provisions or your actual state or country of residence. If any provision of this agreement shall be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this agreement and shall not affect the validity and enforceability of any remaining provisions. This is the entire agreement between the parties relating to the subject matter herein.
