This is an internet website owned and operated by P4 Healthcare, LLC, with its headquarters at 6031 University Blvd., Suite 235, Ellicott City, MD 21043. References to “we,” “our,” “us,”, "this site", "P4", or “P4, LLC” mean P4 Healthcare, LLC, and its subsidiaries, agents, licensors, suppliers, and vendors. References to “you” or “your” mean unregistered visitor(s) and registered user(s) of the this site. References to “registered user” mean a patient (or family member) or a health care professional who has registered to obtain full access of the “Patients and Caregivers” or “Healthcare Professionals” pages of the this site, respectively.

Your Privacy is very important to us. We will not sell, rent, or forward to third parties any individually identifiable personal information (e.g., your name, address, telephone number, e-mail address, etc.) you provide during the registration process or enter into our website unless you have specifically given us the permission to do so or unless required by law. Otherwise, the only third parties that may access your information without your consent are suppliers and vendors that service our website, but only to the extent such service providers have a need for such access in the course of providing services to us. We will obtain confidentiality agreements from such service providers. We may, however, make anonymous by removing or masking personal identifiers and/or aggregating publicly available information, your personal health information, and that of other registered users to provide such information in that form to third parties, but never in such a way that it violates your privacy. Aggregate data does not contain any information that could be used to contact or identify you. Although we are not subject to the requirements of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), we have nevertheless conducted a review of our privacy practices and websites and consider them to be consistent with the requirements of HIPAA.

We are a licensee of the TRUSTe Privacy Program. TRUSTe is an independent, non-profit organization whose mission is to build user’s trust and confidence in the Internet by promoting the use of fair information practices. Information Provided at Registration

This site's registration form requires users to give us contact information (e.g., name, e-mail address), demographic information (e.g., ZIP code, age), and some health condition-specific information to personalize the website experience. No financial information is collected.

We use contact information from the registration form for internal purposes and to send the user news, information about new website features, services, and updates, information about us, and promotional materials or offers from some of our corporate partners. We use demographic and personal health information to tailor the registered user’s experience at our website, showing them content that we think they might be interested in and displaying the content according to their preferences. Your health information is password-protected to prevent access by others. However, we may need to disclose personal information when required by applicable local, state, federal, or international law.

All personal, identifiable information of the registered users is retained by us for a period of five (5) years unless a member requests that the information be deleted. If you would like your information deleted, then you can remove all your records from our website by providing us with your user name and the e-mail address you provided during registration and sending your request to our support group at support@p4healthcare.com. The process of removing your information normally takes three (3) business days, and you will be notified via e-mail upon completion of the removal process. Please note that we cannot delete information that have been released pursuant to your authorization prior to your request or that have been released in anonymous, de-identified, aggregate form.

Choice/Opt-Out

We provide registered users the opportunity to opt-out of receiving communications from our partners and us. When you complete the registration or make changes to your account, you will be given the choice of receiving informational/promotional e-mails from us. You may opt-out of receiving these e-mails by removing the automatic check mark from the preference box for receiving e-mails and newsletters. You may also opt-out of receiving these e-mails and newsletters by changing your preferences in your Account screen at any time.

However, if you choose to remove or not supply us with personal information, then you will be unable to access some of our website features. You will be unable to access personalized features, such as Symptom Tracker, My Health Center or Health Tools, personalized and dynamic content, and ENewsletters. However, you will still have access to our static content.

Notification of Changes

If we are going to use your personally identifiable information in a manner different from that stated at the time of collection, then we will notify you via e-mail. You will have a choice as to whether we can use your information in this different manner. You must affirmatively respond, as instructed in the email notification, if you do not wish for us to use your personally identifiable information in a different manner than is described in this Privacy Policy. In addition, if we make any material change in our privacy practices that do not affect user information already stored in our database, then we will post a prominent notice on our website notifying registered users of the change. We are committed to privacy protection and utilize what we believe to be the most current security tools available to ensure the privacy and confidentiality of all registered users.
