To enter the Honor Your Oncology Nurse, please complete the entry form and submit your essay of up to 500 words. All entries must be submitted online. Entry essays explain why you appreciate a specific oncology nurse.

Current and former patients and caregivers are invited to enter. Limit one entry per person. All entries must be received by June 30, 2008. Selected entries may be published in a 2008 issue of Caring4Urology or on www.Caring4Urology.com and related products in any media, whether now known or thereafter devised, without compensation. A winner will be selected in July 2008 and be notified by phone or e-mail. Selection will be based on inspirational value and ability to honor the nursing profession. The decision of the judges is final.

The winner must agree in writing that P4 Healthcare, LLC may use winner's name, photograph or other likeness, biographical information and statements concerning the contest or the sponsors, and essays for purposes of advertising and promotion without compensation.
